Job Description:

Grace Home Ministries is currently looking for a person to fill the position of Case Manager within their maternity home, The Grace Home, which serves teen moms (ages 13-20) and their children.

 

The Case Manager shall be called of God; committed to serve under the direction of the Program Director to create a Godly Christian home atmosphere and firm but loving relationships, realizing many women come from dysfunctional families and may have problems, deep hurts, and lack of respect for authority.

 

Qualifications:

  1. Committed Christian and active in a local church.
  2. Full agreement with GHM Statement of Faith and Christian Values Statement.
  3. A candidate for the position of Case Manager should at least have:
  • A master’s degree in social work, psychology or counseling;
  • OR a bachelor’s degree in social work or psychology with documented fieldwork experience and must be supervised by the program director or other staff employed by Grace Home Ministries with the aforementioned qualifications;
  • OR a bachelor’s degree and three years of professional experience working with children.
  1. Evidence of a commitment to the sanctity of human life and a heart for pro-life housing ministries. In their attitudes concerning teenage pregnancy, the Case Manager should have a neutral stance on adoption vs. parenting.
  2. Good spiritual, physical and emotional health.
  3. Able to maintain confidentiality for young women and staff.

 

Responsibilities:

  1. Coordination of all services offered to each resident.
  2. Provision of case management services as required by the Virginia Department of Social Services.
  3. Help residents strengthen their capacity to function productively in interpersonal relationships.
  4. Develop and implement a plan of care for each resident with input from the resident, the resident’s family and/or case worker.
  5. Help the resident and the parents or legal guardian understand the effects on the resident of separation from the family and the effect of group living.
  6. Confer with the child care staff to help them understand the residents’ needs in order to promote adjustment to group living, and ensure movement towards goals.
  7. Document and maintain accurate, confidential files on each resident.
  8. Establish, maintain and utilize relationships with other agencies and community resources to provide services.
  9. Refer residents with psychiatric needs to appropriate medical professional.
  10. Work with the resident and with the family or any placing agency that may be involved in planning for the resident’s future and in preparing the resident for the return home or to another family, for independent living, or for other residential care.
  11. Provide support to resident and family to maintain their relationships and prepare for resident’s future care.
  12. Follow up and schedule aftercare counseling for residents after successful completion of the program.
  13. Attend all necessary trainings.
