Job Description:
FOCUS - To provide in tandem with and under the direction of the Program Coordinator, the social work services required by the various counties in their contracts with Children's Choice and to do so in a manner and spirit consistent with the mission statement and core values of Childrens Choice.
ESSENTIAL FUNCTIONS - To maintain a caseload of children and families as assigned by the Program Coordinator and according to contract expectations. To meet with each child according to contract standards. To meet with resource parents at least once a month, ensuring compliance with state and contract regulations and providing support in meeting case planning goals. To meet with birth parents at least once a month, when they are available, and to provide assistance, referrals and support in meeting case planning goals. To initiate permanency planning for a child at placement and work toward permanency goals throughout childs placement. To develop an Individual Service Plan for each child and to assist the members of the treatment team in achieving established goals. To ensure the timely scheduling and completion of medicals, dentals, case sessions, birth family visits, therapy sessions and all other appointments designated in the childs ISP and/or necessary to ensure safety and achieve permanency. To interact with collaborative service providers in order to meet the clients needs. To attend court hearings and service planning sessions, and any other meetings deemed necessary for the case. To maintain accurate, detailed and current case records on each client; preparing quarterly reports according to established deadlines, as well as other reports required by the contracts, documenting case sessions and contact notes, including date, time, and signature of the recorder. To assume emergency duty as assigned, providing immediate assistance, including on-site presence during a crisis, relaying relevant information to the Program Coordinator/Project Supervisor and the clients caseworker. To attend all scheduled staff meetings, supervisory conferences, and in-service training sessions. To relay summary information on each client to the Program Coordinator/Project Supervisor and his/her appointed designee at regularly scheduled supervisory meetings and informally as necessary. To analyze client needs and develop appropriate interventions. To plan and conduct group meetings or trainings as scheduled or assigned by the Program Coordinator/Project Supervisor.
REQUIREMENTS - BSW with licensure. Experience working with children or families in social services context. Possess a valid driver's license with properly registered and insured vehicle with back seat appropriate for the transportation of children. Able to obtain appropriate Child Abuse and Criminal Record Clearances. Proficient in Microsoft Office.
